Office Procedures

To our new patients

We are pleased to have you as our patient. Please read the following information to become familiar with our office policies.

 

Office Procedures

  • Our normal office hours are Monday through Thursday, from 9am to 5pm, and Friday from 9am to 4pm. We will on occasion see patients as early as 8:30am.
  • We see patients by appointment only. Occasionally, emergencies do arise which detain the doctor. We ask you to be patient and understanding. We will work hard to give you prompt attention.
  • When you arrive for your appointment, we ask that you 1) Sign in, 2) Have a seat and wait until called, and 3) If you have x-ray or questions for the nurse or doctor, please hold those until your are seen.
  • In consideration of our asthma and allergy patients, please do not wear any perfume, cologne, lotion or after shave to the office. This restriction also applies to family members who may be accompanying you.
  • If you can't keep your appointment for any reason, it is your responsibility to notify us at least 24 hours in advance. If we are not notified, you will be billed a $50 missed appointment fee. Missed appointment fees are subject to pre-payment charge prior to rescheduling and cannot be filed on insurance.
  • If you are filing disability forms, please expect 7 to 10 working days for these to be completed and returned. There is a $20 fee for this service, payable in advance.
  • If there are questions, comments, or complaints, please direct them to our Practice Manager, Patricia Plummer.

Payment

Insurance and Other Payments

  • Payment is expected at the time of service. If you are not prepared to pay your co-payment at the time of the appointment, you may be refused service.
  • We accept checks, MasterCard, VISA, Discover, Diners Club and American Express. If, for any reason, it is necessary to bill you for the visit, there will be a $10 administrative fee added to the bill.
